For a hassle free stay in our facilities, please verify your PhilHealth contributions and coverage through our Billing/Philealth Section.

Requirements: PhilHealth Identification Card or Card Number

If you are not a member of Philhealth, please proceed with our PhilHealth Officer to assist you during your stay.

Global Care Medical Center of Bay recognizes the fact that hospitalization cost is always a concern for most patients. Fortunately, GCMCB is one of the PhilHealth-accredited hospitals which exert maximum effort to provide best care and assistance to all PhilHealth members towards the attainment of their benefits. 

   As a PhilHealth member, you and your dependents are entitled to health and hospitalization subsidies. All you have to do is go to our PhilHealth Section, which is located at the ground floor near the Cashier, and fill out an information slip. Your provided information will be keyed in by our PhilHealth Officer to an online portal to validate PhilHealth membership or dependency and premium contribution. Once the portal yields a YES as an answer to the aforementioned, then the member/dependent is eligible to avail of PhilHealth benefits for the said confinement. 

If the portal says NO, the member will then be asked to provide the following documents:

1. Duly accomplished and signed Claim Signature Form (CSF);
2. Certificate of Contribution from Employer or Premium Payment (for at least 9 months within 12-month period inclusive of the confinement month of the patient);
3. Member Data Record (MDR);

* In case the MDR does not reflect the name/s of the member’s qualified dependents, the following additional requirements must be submitted:

1. Duly accomplished and signed PhilHealth Membership Registration Form (PMRF);
2. Proof of dependency (i.e., Marriage Certificate, Birth Certificate) – if the patient is a qualified dependent.

Note: In case our PhilHealth portal is offline, we might require you to submit the same documents listed above for you to avail your PhilHealth benefits.

Day-to-day operations of our PhilHealth Section are from Monday to Sunday, 8 a.m. until 5 p.m.
