I. The rights of patient at Global Care Medical Center of Bay are as follows (Ang Karapatan ng pasyente sa Global Medical Center of Bay ay ang mga sumusunod):

  1. The responsibility to provide information (Tungkuling sabihin ang tama at kumpletong impormasyon tungkol sa sakit ng pasyente).
  2. The right to informed consent. (Karapatang mabigyan ng malinaw at makatotohanang paliwanag sa pamamaraang kanyang mauunawaan ang lahat ng impormasyon tungkol sa pagsusuri, eksamen, gamutan o operasyon na gagawin sa pasyente at karapatang hingin ang kanyang pahintulot bago ito isagawa).
  3. The right to privacy and confidentiality. (Karapatang maging pribado ang usapan tungkol sa kanyang sakit).
  4. The right to be informed. (Karapatang malaman ang tunay nyang sakit at lahat ng resulta ng kanyang eksaminasyon, lahat ng mga bagay patungkol sa kanyang karamdaman at pati na ang mga bayaning kanyang haharapin.)
  5. The right to refuse and to self-determination.(Karapatang tumanggi sa gamutan o mabigyan ng pangunahing lunas matapos maipaliwanag ang magiging kahinatnat nito. Karapatang tumaggi sa anuman uri ng eksperimento/ pagsusuri o pag-aaral).
  6. The right to choose other health care provider and facility. (Karapatang mamili ng kanyang doktor o pagamutan, humingi ng pangalawang opinion at malaman kung may alternatibong paraan ng gamutan sa kanyang sakit).
  7. The right to religious belief. (Karapatang tumanggi sa gamutan kung hindi sang-ayon sa kanyang paniniwala matapos maipaliwanag sa kanya ang benepisyo at peligro nito).
  8. The right to leave. (Karapatang umuwi o umalis ng ospital matapos maipaliwanag ang magiging kahinatnan ng kanyang desisyon).
  9. The right to express grievances. (Karapatang maghain ng hinaing kung may pagkukulang ang ospital. Maaring magtungo sa tanggapan ng Administrative Officer/Hospital Adminitrator)
  10. The right to know hospital rules and regulations (Karapatang malaman ang mga alituntunin ng ospital).

II. Patient Responsibilities (Mga Tungkulin ng Pasyente):

  1. The responsibility to provide information (Tungkuling sabihin ang tama at kumpletong impormasyon tungkol sa sakit ng pasyente).
  2. The responsibility to follow treatment plan as recommended by his physician. (Tungkuling sumunod sa mga pinag-uutos at pinapayo ng doktor at makibahagi sa pagpapasya ukol sa kalusugan ng pasyente.)
  3. The responsibility to report the unexpected changes in his medical condition to his physician. (Tungkuling sabihin sa doktor ng pasyente kung may hindi inaasahang pagbabago tungkol sa kondisyon ng pasyente).
  4. The responsibility to ask questions regarding the nature of the patient’s illness and its management including pain relief options as outlined by his physician. (Tungkulin magtanong ukol sa tunay na kalagayan ng karamdaman ng pasyente, mga posibleng lunas,at  komplikasyon ng kanyan sakit.)
  5. The responsibility to inform the physician of refusal of treatment and notify them immediately if he is unable to do the plan of care which is agreed upon. (Tungkuling sabihin sa doktor ng pasyente kung hindi makakasunod sa gamutan o problema sa pagsunod sa napagkasunduang plano ng gamutan).
  6. The responsibility to pay his financial obligations to the hospital and his health care providers are fulfilled as promptly as possible. (Tungkuling bayaran ang pinansyal na obligasyon sa ospital at sa doktor).
  7. The responsibility to comply with the hospital general rules. (Tungkuling sumunod sa mga alituntunin ng ospital).
  8. The responsibility to be considerate to the needs of other patients and hospital staff. (Tungkuling isaalang-alang at igalang ang mga karapatan ng ibang pasyente at mga kawani ng ospital at bigyan ng kosiderayon ang kapwa pasyente tungkol sa pagtanggap ng panauhi, at pag-iingay).

 

I hereby attest and I have full understanding in my rights and responsibilities stated herein accordance with the Global Care Medical Center of Bay Policies and Procedure. (Ako ay lubos na sumusumpa, na aking nauunawaan ng mabuti ang Karapatan at Tungkulin ng pasyente sa Global Care Medical Center of Bay na ayon sa kanilang Policies and Procedure.)
